• Health Wise Acupuncture is open 5 days a week from 8am to 6pm.

• Complimentary consultations are 15 minutes

• A new patient visit is 1.5 hours for health history and treatment.

• An established patient visit is 1 hour.

• Appointments are booked on the hour or half hour throughout the day.

• 24 hour cancellation notice is required.

If you are a new patient please download the following forms fill them out and bring them with you to your appointment. If you have any medical history records you feel are pertinent to your concern please bring a copy of them with you.

Payment for services is expected at time of service. As a convenience for our patients Health Wise Acupuncture will bill your insurance company if you have coverage for our services. Please check with your insurance plan to verify that you do. To contact your plan see your insurance ID card. On the backside of the card there will be a phone number to call and inquire about coverage.
